Abstract
Objective To improve, in the regional healthcare system, standards of care of patients suffering of Stroke through implementation of integrated clinical pathways (ICPs) and development of a ‘Regional Audit Program’.
Programme
Establishment of multidisciplinary and multiprofessional regional expert panels with the mission of selecting and sharing the best evidence-based interventions for Stroke, setting indicators and standards to assess performances and outcomes.
Creation of multidisciplinary and multiprofessional local workteams (one for each Local Health Authority and hospital) with the aim of implementing recommendations in daily practice using integrated care pathways (ICPs) as an effective tool to reach integration at different levels of care and professional competences.
Development of a ‘Regional Audit Program’ incorporating the selected indicators to measure the performances of the whole regional healthcare system and the patients outcomes.
Elaboration of reports on the state of healthcare quality delivered for Stroke.
Auditing and benchmarking activities at central and local levels to analyse the results of the reports and review activities with low performance values.
Programme
Results From data collected in the monitoring system we generated two reports. Results refer to years 2006 and 2007. Data collection was conducted in eight out of 15 hospitals of Marche Region, for two consecutive months, random selected, both in 2006 and 2007 (182 patients in 2006, 178 patients in 2007). We have summarised results of a sample of the most representative indicators in the following table.

Discussion, conclusion The program has demonstrated to work, enabling the system to assess quality of care delivered to patients suffering of Stroke. Although the results are still limited, they show a good performance on assessment, prevention of complications (apart from pressure ulcers), medications and secondary prevention but still an healthcare quality far from standards in the field of multiprofessional integration, patients/caregiver education and provision of rehabilitation. On the basis of such results, activities of organisational and clinical audit and benchmarking were undertaken and several actions were planned to remodel interventions and organisational systems to achieve standards of care for all patients with stroke within Marche Region.
In December 2009 we'll collect data from about 500 patients from all the 15 hospital of Marche Region to prepare the third report of the Stroke Marche Regional Audit Program.
Contexte, objectif(s) Améliorer, dans le système régional de santé, les standards de prise en charge des patients atteints d'infarctus cérébral par la mise en place de chemins cliniques intégrés et le développement d'un « Programme Régional d'Audit »
Programme
Création d'un groupe d'experts régional, multidisciplinaire et multiprofessionnel, ayant pour mission de sélectionner et de partager les meilleures interventions fondées sur les preuves (evidence-based) pour l'infarctus cérébral, en définissant des indicateurs et des standards pour évaluer les performances et les issues.
Création de groupes de travail locaux multidisciplinaires et multiprofessionnels (un par zone territoriale et par hôpital) ayant pour objectif de mettre en œuvre des recommandations pour la pratique quotidienne en utilisant les chemins cliniques comme outil effectif pour permettre l'intégration des différents niveaux de prise en charge et améliorer les compétences professionnelles.
Développement d'un « Programme Régional d'Audit » comprenant les indicateurs sélectionnés afin de mesurer les performances de tout le système régional de santé et les issues des patients.
Élaboration de rapports sur le niveau de qualité garantie pour l'infarctus cérébral.
Activités d'audit et de benchmarking au niveau régional et local afin d'analyser les résultats des rapports et revoir les pratiques ayant obtenu de faibles performances.
Programme
Résultats Deux rapports ont été produits à partir des données collectées par le système de contrôle. Les résultats portent sur les années 2006 et 2007. Les données ont été collectées dans 8 des 15 hôpitaux de la Région des Marches, par sélection aléatoire, durant deux mois consécutifs (182 patients en 2006, 178 patients en 2007). Dans le tableau suivant, nous avons synthétisé les résultats d'un échantillon des indicateurs les plus représentatifs.

Discussion, conclusion Le programme a montré qu'il fonctionne en permettant au système d'évaluer la qualité de la prise en charge des patients souffrant d'infarctus cérébral. Bien que les résultats soient encore limités, ils enregistrent de bonnes performances dans l'évaluation, la prévention des complications (mis à part pour les ulcères de pression), la médication et la prévention secondaire, mais montrent une qualité de soins encore loin des standards en matière d'intégration multiprofessionnelle, d'éducation patients/aidant naturel et d'offre de rééducation.
Sur la base de ces résultats, des activités d'organisation, d'audit clinique et de benchmarking ont été entreprises et plusieurs actions ont été planifiées pour revoir interventions et systèmes d'organisation dans le but d'atteindre les standards de prise en charge pour tous les patients souffrant d'infarctus cérébral dans la Région des Marches. En décembre 2009, nous collecterons des données concernant environ 500 patients et les 15 hôpitaux de la Région des Marches, afin de préparer le troisième rapport consacré au Programme Régional d'Audit sur l'Infarctus Cérébral.
