COPDTo reduce by 30% the number of hospital admissions (compared with the previous 12 months) for respiratory illness in patients with COPD.
Chronic disease patient self-managementTo increase the identification of those with risk factors for chronic disease, and implement strategies to mitigate these risks, including self-management. Through this, the aim was to assess risk factor status in 50% of those in whom it is recommended, and reduce the number of risk factors that are not at target by 20%.
Closing the gap (aboriginal health)To achieve a 30% improvement in the number of health assessments for Aboriginal and/or Torres Strait Islander peoples, performed by health services over the last 12 months.

  • 50% of patients with diabetes types 1 and 2 within participating health services should have an HbA1C of 7% or less
