An innovative model to coordinate healthcare and social services for people with serious mental illness: A mixed-methods case study of Maryland's Medicaid health home program
2018, General Hospital PsychiatryCitation Excerpt :PRP directors reported difficulty finding nurse care managers interested in performing population health management tasks as opposed to delivering direct clinic care. Leaders also noted that the reimbursement requirement to deliver two health home services for all participants with SMI per month, including those in good health, sometimes prevented them from implementing population health management best practices [67–69] related to prioritizing high-need consumers. Leaders reported shifting the roles of existing PRP staff to support health home implementation, for example by having PRP staff request medical records from external primary care providers.
Why is major depression prevalence not changing?
2016, Journal of Affective DisordersCitation Excerpt :Effective treatments for depression should lead to diminished prevalence if they reduce the recurrence of MDE (e.g. during the long-term use of antidepressant medications) or if they reduce the mean duration of episodes during acute treatment. The biggest impact on population health can be expected to arise from the former rather than the latter effect (Katon et al., 1997; Vos et al., 2004). In view of evidence of increased treatment, a lack of improvement in prevalence is therefore puzzling.
