3Operative consent incomplete, missing, or otherwise incorrect
4Unplanned removal, injury, or repair of organ or structure during surgery, invasive procedure, or vaginal delivery
5Unplanned return to operating room on this admission
6Invasive procedure with tissue removed where pathology report does not match preoperative diagnosis, or non-diagnostic or no tissue removed
7Transfusion required for bleeding/anaemia/other iatrogenic reason, not clinically indicated, or resulting in reaction
8Nosocomial (hospital acquired) infection
9Antibiotic or drug use problems
10Cardiac or respiratory arrest, or low Apgar score
11Transfer from general care unit to special care unit
12Other patient complications
13Hospital incurred patient incident, such as fall, intravenous problem, medication error, or skin problem
14Abnormal laboratory, radiograph, or other test results not addressed by physicians
15Neurological deficit present at discharge which was not present on admission
