  • The result of the primary analysis at 12 months is displayed at the top (intention to treat). Indicator scores were calculated by dividing the sum of fulfilled criteria by the total number of criteria per indicator, resulting in a score between 0 and 1; ‘0’ denotes ‘no criterion achieved’ and 1 ‘all criteria achieved’. Mean and SD are presented. The description of criteria, with scores displayed depending on the number of fulfilled criteria, is shown further down (Note: Results of complete cases are displayed). Criteria 2–6 are self-reported, criterion 1 is assessed by chart review.
