Knee and hip osteoarthritis
  • Diagnosis of knee or hip osteoarthritis

  • Age 21 or older

  • Prior partial or total knee or hip replacement surgery

  • Hip fracture or aseptic necrosis in 12 months prior to visit

  • Rheumatoid arthritis or psoriatic arthritis

  • Overall health makes surgery inappropriate.

Herniated disc
  • Back and leg pain lasting more than 4 weeks

  • Imaging confirmed diagnosis of herniated disc.

  • Age 21 or older

  • Presence of symptoms (eg, severe weakness in legs, change in bowel or bladder function) that indicate more serious condition

  • Previous back surgery

  • Overall health makes surgery inappropriate.

Spinal stenosis
  • Back and leg pain lasting more than 8 weeks

  • Imaging confirmed diagnosis of spinal stenosis.

  • Age 21 or older

  • Presence of symptoms (eg, severe weakness in legs, change in bowel or bladder function) that indicate more serious condition

  • Previous back surgery

  • Overall health makes surgery inappropriate.
