Herniated disc	Back and leg pain lasting more than 4 weeks Imaging confirmed diagnosis of herniated disc. Age 21 or older	Presence of symptoms (eg, severe weakness in legs, change in bowel or bladder function) that indicate more serious condition Previous back surgery Overall health makes surgery inappropriate.
Spinal stenosis	Back and leg pain lasting more than 8 weeks Imaging confirmed diagnosis of spinal stenosis. Age 21 or older	Presence of symptoms (eg, severe weakness in legs, change in bowel or bladder function) that indicate more serious condition Previous back surgery Overall health makes surgery inappropriate.
