Web-based PRISM toolInstalled on computers in each practice and activated when the practice began the intervention.
Practice-based trainingTwo-hour session delivered in the practice by a GP champion to the nominated lead GP for PRISM, the practice manager and any other interested staff.
GP championsTwo local GPs employed for two sessions per month to support practices in clinical use of PRISM.
Technical help deskTelephone and email support provided in office hours by NHS Wales Informatics Service (NWIS) to answer enquiries about technical aspects of using PRISM.
PRISM handbookUser friendly 25-page handbook explaining how to set up and access PRISM, demonstrating the range of functions available in PRISM and recommending how to use it within the practice.
Targeted carePractices were free to plan tailored care for patients at high risk. QOF payments required identification and active management of patients at high risk.
