Ventilatory functions of normal children and young adults—Mexican-American, white, and black. I. Spirometry

This is a study of the ventilatory functions of 1,805 normal Mexican-American, white, and black students of six public schools in Houston, Texas, with ages ranging from 7 to 20. A roll-seal piston type spirometer was used. The best performed forced vital capacity curve of each student was selected by the computer program from which the following measurements were extracted: FVC, FEV1, FEV1/FVC ratio, PEFR, and MMEF. Each student also had the peak expiratory flow rate measured by the Wright peak flowmeter to establish normal values with this instrument. Significant differences of lung volume and flow rate exist among the three races, and between male and female subjects. Prediction equations and prediction curves for each race and sex are presented. The results of the present study are compared with those of previously published works.

    The majority of that research, however, was not designed with the aim of producing spirometric-prediction equations (Kiyamu et al., 2012; Frisancho et al., 1997; Weitz et al., 2016; Kiyamu et al., 2015), while other studies had limitations that made the results unreliable for routine clinical use (Rojas and Dennis, 2010; Santolaya et al., 1984). As a consequence, spirometric studies performed on those subjects are interpreted and reported using reference equations generated at low altitude (Quanjer et al., 1993; Hankinson et al., 1999; Roca et al., 1998; Hsu et al., 1979; Crapo et al., 1981,1990; Quanjer et al., 2012; Galíndez et al., 1998; Pérez Padilla et al., 2006). The American Thoracic Society (ATS) and European Respiratory Society (ERS) recommend studying a healthy sample of the local community in an attempt to validate published pulmonary function reference equations (Miller et al., 2005a,b; Pellegrino et al., 2005).

    The currently recognized noninfectious pulmonary manifestations of chronic GVHD are bronchiolitis obliterans syndrome (BOS) and its earlier precursor, which is lesser degrees of airflow obstruction [65]. Dyspnea, wheeze, or cough occur frequently in chronic GVHD and clinical examination, pulmonary function tests (PFTs) [66-68], radiologic studies, and bronchoalveolar lavage help to distinguish BOS from other pulmonary diseases (eg, pulmonary thromboembolism, cryptogenic-organizing pneumonia, radiation-induced lung injury), extrapulmonary diseases (eg, effusions, subcutaneous sclerosis), and lung infections. Air trapping and bronchiectasis may result from advanced BOS, infections, or both.
